Job summary

Colonial Group in Hudson, MA is seeking an experienced LPN to join our compassionate team. The LPN will provide hands-on nursing care, supervise Resident Care Assistants, and ensure accurate medication administration and documentation in the EHR system.

The ideal candidate holds an Associate’s or Bachelor's degree in Nursing, with a minimum of three years in long-term care and at least one year in a supervisory capacity. Massachusetts RN/LPN license required.

Qualifications

• Associate’s or BSN required.
• Minimum of three years’ experience in personal and/or long term care with at least one year in a supervisory role.
• Knowledge of medication administration and regulatory procedures; strong organizational and communication skills.
• Licensed Registered Nurse or Licensed Practical Nurse in Massachusetts.

Responsibilities

• Provide care, safe environment, and emotional support to residents.
• Supervise Resident Care Assistants, provide education and evaluations.
• Administer medications and treatments and ensure proper documentation.
• Document care in the Electronic Health Records (EHR) system.
• Ensure HIPAA-compliant documentation and reporting of resident concerns.
