Job summary

Lowe’s Guardian Angel is seeking a Part-Time Licensed Practical Nurse (LPN) to provide medical oversight for individuals with IDD in residential and day-support settings.

Under the Nursing Director, you will coordinate medical care, manage documentation, and ensure DBHDD standards and compliance. This role requires strong clinical judgment, organization, and effective communication with staff and families.

Qualifications

• Current and unrestricted LPN license in Georgia.
• Current CPR/BLS certification.
• Experience with intellectual/developmental disabilities or dual diagnoses preferred.
• Knowledge of DBHDD standards and medication management preferred.
• Strong clinical documentation, communication, organizational and time-management skills.
• Ability to work independently while collaborating with leadership and staff.
• Valid driver’s license and transportation when travel is required.

Responsibilities

• Coordinate medical care and provide clinical oversight for IDD/dual-diagnosis clients in residential and day-support settings.
• Complete and oversee medical documentation and MA/HRST-related reporting.
• Monitor quality and continuity of medical services and coordinate with direct-care staff.
• Attend ISP and SIS meetings as assigned and assist with intake/transition processes.
• Educate and train staff on medical-management procedures and regulatory requirements.
• Audit MARs and medical records for accuracy; ensure timely follow-up and referrals.
