Job summary

InCommunity is seeking an LPN to provide direct nursing care to individuals with intellectual and developmental disabilities in community settings. The role includes monitoring vital signs, administering medications, and coordinating with physicians to ensure high-quality care.

The position requires current Georgia LPN licensure, CPR and First Aid certification, and willingness to work a flexible schedule including on-call shifts in community homes.

Qualifications

• Current, valid LPN license in Georgia.
• CPR and First Aid certification (or ability to obtain upon hire).

Responsibilities

• Provide direct nursing care, including monitoring vital signs and assessing health status.
• Administer medications and treatments per physician orders and regulatory standards.
• Maintain accurate documentation of nursing care in medical records.
• Assist in developing and implementing person-centered care plans.
• Provide first aid and emergency care and coordinate with physicians and other professionals.
• Educate individuals and families on health topics including hygiene and preventive care.
• Advocate for rights and dignity of individuals and ensure culturally sensitive care.
• Collaborate with direct support professionals and interdisciplinary team members.
• Ensure compliance with regulations, policies, and healthcare standards.
• Participate in meetings, training, and ongoing professional development.
• On-call rotation and occasional evening/weekend involvement as needed.
