Job summary

Amedisys, Inc. is seeking a licensed practical nurse for part-time home health visits in Augusta, GA. You will provide compassionate in-home patient care, collaborate with the care team, and ensure plans of care are followed during each visit.

Qualifications

• One year of clinical LPN experience.
• Current LPN license for the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to address patient needs.
• Teach patients and caregivers self-care techniques.
• Check vital signs at each visit.
• Review medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (i.e. foley catheter change).
• Prepare equipment and materials for treatments using the aseptic technique.
• Complete documentation timely and accurately.
