Job summary

Amedisys is seeking a licensed practical nurse for full-time days in Georgia, delivering home health services to patients in their homes. The role includes performing nurse visits, coordinating with RNs and physicians, and educating patients and families.

Qualifications

• LPN with at least one year of clinical experience.
• Active LPN license in assigned state.
• Current CPR certification.
• Valid driver’s license with reliable transportation.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify changes promptly.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform reconciliation at each visit.
• Perform physician-ordered procedures (e.g., Foley catheter changes).
• Prepare equipment and materials using aseptic technique.
• Complete documentation timely and accurately.
