Job summary

Jaybird Senior Living in Illinois seeks a part-time Licensed Practical Nurse for first or second shift to provide medical treatment and personal care to residents under the Health Care Coordinator's supervision, ensuring safety, comfort and well-being.

Responsibilities include administering medications and treatments with accurate documentation in the Yardi EHR, taking vital signs, assisting with assessments and incident reporting, preparing diet trays, maintaining room safety, and supporting

Qualifications

• LPN license in good standing with the state.
• Minimum six months caregiving experience.
• Strong interpersonal skills; professional and compassionate.
• Ability to work in a multidisciplinary team.
• Physically able to lift up to 50 lbs.

Responsibilities

• Administer medications and treatments; document accurately in EHR (Yardi).
• Take vital signs and monitor resident condition.
• Assist with resident assessments, nursing calls, and incident reporting.
• Collect lab samples and follow up on results with residents, families, and physicians.
• Prepare and check food trays per prescribed diets.
• Maintain cleanliness and safety in resident rooms; respond to resident calls.
• Support and train staff as directed by HCC.
• Comply with HIPAA, company policies, and federal/state regulations.
