Nexus Family Healing is seeking a part-time Licensed Practical Nurse (LPN) for the Southeast Regional Crisis Center in Rochester, MN. The role focuses on delivering specialized medical services to individuals in crisis, with a commitment to equity and community collaboration.

The LPN will monitor health conditions, coordinate referrals, and maintain records while working a flexible schedule between 7am and 11pm, with some weekends.

Qualifications

• Graduate of an accredited school of Practical Nursing.
• 2+ years' experience in a clinic and/or hospital setting.
• Valid driver's license and DHS background check.
• Must meet cultural needs of diverse communities.

Responsabilités

• Provide client-centered medical care and monitor health conditions.
• Assess and respond to medical concerns, coordinate referrals.
• Perform nursing assessments and support admissions process.
• Maintain accurate medical records and documentation.
• Collaborate with physicians and treatment teams for care plans.
• Coordinate follow-up appointments and ongoing medical services.
• Administer, monitor, and document medications per policies.
• Educate clients on medications, side effects, and interactions.
