Nexus Family Healing in Rochester, MN is seeking a Part-time Case Manager (20 hours/week) for our Southeast Regional Crisis Center. This on-site role offers a starting salary of 25–27 USD per hour and involves a mix of days/evenings/weekends.

You will coordinate therapeutic services, connect clients with community resources, and support discharge planning while maintaining confidentiality and high professional standards.

Qualifikationen

• Bachelor's degree required or equivalent experience in social work/case management.
• 5+ years of experience in social work, case management or similar duties.

Aufgaben

• Conduct client intake, assessments, and treatment planning to support stabilization.
• Connect youth, adults, and families with community resources and supports.
• Develop and coordinate discharge and aftercare plans with follow-up services.
• Provide crisis intervention, advocacy, and resource navigation for clients and families.
• Collaborate with schools, healthcare providers, social services and community partners.
• Maintain accurate documentation and ensure compliance with policies.
