Liberty Community Living Center is seeking a Life Connections Activities Assistant to help plan and deliver engaging programs for residents under the Life Connections Coordinator. This part-time role emphasizes active participation, safety, and teamwork with care staff.

Responsibilities include leading individual and group activities, maintaining records, promoting Residents’ Rights, and following infection control and safety procedures while attending required orientations and trainings.

Qualificações

• High school diploma or equivalent required.
• No prior experience or training needed.
• Long term care experience preferred.

Responsabilidades

• Assist in planned activities based on assessment and care plan.
• Conduct and/or assist in individual, group and bedside activities.
• Work with staff to ensure resident participation.
• Assist in maintaining required records and documentation.
• Follow Residents’ Rights, safety, infection control, and isolation procedures.
• Attend orientations, in-services, and trainings as requested.
• Attend staff meetings as requested.
