Job summary

HealthDrive is seeking a Licensed Independent Clinical Social Worker (LICSW) to join our part-time team, working two days per week across nursing facilities in the MetroWest/Central MA area, including Marlborough and Westborough. Qualifications

• Master's degree from an accredited college or university.
• State professional LICSW license.
• Valid driver’s license.
• Ability to work independently on a daily basis.
• Excellent written, verbal, interpersonal and organizational skills.
• Ability to use email, the internet, and to learn other healthcare related software.

Responsibilities

• Complete psychosocial (initial) assessments
• Provide individual and group therapy
• Develop behavior plans for residents
• Recommend behavioral/therapeutic interventions to nursing home staff
• Enters patient information and required clinical documentation in EMR system
• Perform other duties as assigned
• Mobile model treatment of patients requires daily travel to skilled nursing facilities
