UPLIFT HOMECARE, LLC is hiring a Licensed Physical Therapist (PT) or Physical Therapist Assistant (PTA) for part-time home health in Lake Wales, Frostproof, and surrounding areas in Florida. You will provide one-on-one in-home therapy, tailor interventions, and collaborate with a clinical team to enhance mobility and independence.

Responsibilities include evaluating function, documenting progress, and educating patients and caregivers.

Qualifications

• Must hold a Florida PT or PTA license.
• Graduate of an accredited PT/PTA program.
• Successful completion of the national licensure examination.
• Minimum 1 year of home health experience preferred.
• Current CPR certification and valid driver's license.

Responsabilités

• Provide therapeutic exercises, treatments, and interventions to improve mobility and function.
• Monitor patient progress and response to treatment.
• Assist with evaluating functional abilities and updating care plans as appropriate.
• Complete clinical documentation accurately and within required timeframes.
• Communicate effectively with patients, caregivers, and the interdisciplinary care team.
• Educate patients and caregivers on exercises, mobility, safety, and treatment goals.
• Participate in care coordination and quality improvement activities.
• Maintain current knowledge of applicable physical therapy practices and standards.

Formation

Florida PT or PTA license

Graduate of an accredited PT/PTA program

Successful completion of the required national examination

Current CPR certification
