Qualifications

• Master's Degree in Social Work from an Accredited Council on Social Work Education program.
• California Clinical Social Worker License upon hire.
• 2 years SW experience in any healthcare setting.
• 1 year experience in acute care setting or successful completion of TIPs program or Case Management Orientation Program.

Responsabilités

• Provide professional, comprehensive patient-centric care management services for at-risk patients in an acute care environment.
• Assess and plan, coordinate care, discharge planning, document interventions, ensure regulatory compliance and advocate for patients.
• Identify and address social, financial, cognitive/behavioral or legal barriers to accessing resources and care to ensure continuity of care.
