Qualifications

• California Fire and Life Safety Card - National Organization within 30 days of hire.
• 2 years of SW experience in any healthcare setting.
• 1 year of experience in acute care setting or successful completion of TIPs program or Case Management Orientation Program.

Responsibilities

• Provide professional, comprehensive patient-centric care management services for at-risk inpatients in an acute care environment.
• Perform assessment and planning, coordinate care, and discharge planning.
• Document interventions and ensure regulatory compliance and patient advocacy.
