Qualifications

• Associate degree with 2+ years of case management or equivalent experience.
• High school diploma with 4+ years of case management experience acceptable.
• Bachelor’s degree preferred in human services or related field.

Responsibilities

• Provide case management services for youth and young adults in transition.
• Conduct intake to establish program eligibility and service needs.
• Coordinate housing stability, budgeting, education, and employment supports.
• Record and maintain case management data and document services.
• Deliver life skills classes and learning opportunities in Sheridan and Johnson counties.
• Perform outreach and referrals with community providers and agencies.
