Qualifications

• Active, unrestricted MD or DO license in the US.
• Ability to travel locally and within licensed states.
• Experience with in-home patient evaluations.
• Strong communication and empathy with patients.

Responsibilities

• Conduct in-home health evaluations (~45 minutes) using an iPad workflow.
• Review medication history and current plan with patients.
• Coordinate care with health plans and primary care providers as needed.
