Responsibilities

• Provide in-home health evaluations and patient education.
• Engage with patients in their homes and review medical history and medications.
• Perform physical evaluations and note findings in the clinical workflow.
• Coordinate with health plans and care teams for referrals and follow-ups.
• May conduct virtual visits as needed and travel within licensed states.
