Signify Health, part of CVS Health®, is seeking a Part-time Nurse Practitioner to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and optional diagnostic tests as ordered by the health plan. Travel locally within licensed states is possible.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.

Responsibilities

• Visit patients in their homes to conduct health evaluations.
• Review medication and medical history and perform a physical evaluation.
• Travel locally or within licensed states; conduct virtual visits as needed.
• Provide Signify Health services including diagnostic and preventive tests as ordered.
