Job summary

Northeast Healthcare Recruitment, Inc. (NEHCR) seeks Nurse Practitioners to perform in-home Health Risk Assessments across King County, WA. This part-time role offers flexible hours and visits seven days a week, with a minimum commitment of 8–10 hours weekly.

Responsibilities include conducting HRAs, documenting conditions, reviewing medications, and educating patients and families on preventive care. WA NP license and reliable transport are required.

Qualifications

• Active Nurse Practitioner license in WA.
• Experience with Health Risk Assessments (HRA) preferred.
• Strong clinical assessment and documentation skills.
• Ability to work independently and manage schedule efficiently.
• Reliable transportation for in-home visits.

Responsibilities

• Conduct comprehensive in-home Health Risk Assessments (HRAs).
• Identify and document acute and chronic medical conditions.
• Obtain vital signs and calculate BMI.
• Review medications and pharmacological therapy.
• Perform a physical examination.
• Evaluate patients’ physical, mental, and social health needs.
• Educate and counsel patients and their families on health conditions and preventative care.
