Job summary

Source One Management Services, LLC seeks Nurse Practitioners for flexible PRN in-home Health Risk Assessments for Medicare Advantage members in Richmond, KY. You’ll assess health status autonomously with a user-friendly EMR, traveling locally to patients and building care snapshots without ongoing treatment responsibilities.

As a 1099 contractor, you may deduct mileage. Onboarding is quick—3 to 4 weeks—and a strong license plus willingness to travel within ~60 minutes of home are required.

Qualifications

• Active NP license (excluding Pediatrics, PMHNP and Women’s Health)
• Minimum eight full 7.5-hour days per month
• Willingness to travel within roughly 60 minutes of home
• Ability to conduct in-home Health Risk Assessments
• Travel locally to patients in your county and nearby areas

Responsibilities

• Complete comprehensive in-home HRAs for Medicare Advantage patients
• Review patient medical history, medications, lifestyle, and social factors
• Conduct basic physical and mental health evaluations
• Travel locally to patients in your county and nearby areas
