Job summary

Signify Health, part of CVS Health, seeks Part Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations. Providers will visit members in their homes, review medications and history, conduct physical evaluations, and perform/coordinate optional diagnostic tests as directed.

The role involves flexible local travel, virtual visits when needed, and offering Signify Health services such as spirometry and bone density screening.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.
