Qualifications

• Board certification is required.
• Multi-state licenses are preferred.

Responsibilities

• Be the in-home clinician for health plan members, delivering compassionate care.
• Perform home visits, reviewing medications and medical histories with patients.
• Conduct physical evaluations and, when ordered by the health plan, include diagnostic tests as appropriate.
• Collaborate with the Signify Health team to coordinate care pathways.
• Travel locally and within licensed states to visit members as needed.
• Offer virtual visits when required by the care plan.
