Qualifications

• Demonstrate ability to read, write, and follow a written Plan of Care and document tasks performed.
• Be self-directed and work with minimal supervision.
• Demonstrate dependability, tact, patience, and the ability to follow instructions.
• Demonstrate good interpersonal communication skills.
• Experience in cleaning, laundry, medication reminders, escort services.

Responsibilities

• Observation of client functioning and reporting changes to his/her supervisor or employer.
• Attends to the client's requests promptly.
• Assistance with activities of daily living and personal care.
• Provides companionship, socialization, and housekeeping services.
• Assists with medication reminding, including prompting to take medications and handling reminder containers.
• Assists with meal planning, purchase of food, and food preparation.
• Maintains a clean and safe environment, including bathrooms and kitchen.
• Handles personal errands and laundry.
• Vacuuming and dusting; changing bed linens.
• Follows infection control policies and communicates with supervisory staff.
• Documents care given and observations accurately.
• Treats clients with respect and maintains confidentiality.
• Attends required in-service programs.
