Job summary

Alc-Health-Care-Services is seeking a part-time, in-person Caregiver in Illinois to support clients in their homes. You will assist with activities of daily living, provide companionship, and help with light housekeeping while following the written Plan of Care and reporting changes to supervisors.

The role requires reliability, good communication, and the ability to work with minimal supervision. Prior caregiver experience is preferred.

Qualifications

• Read, write, and follow a written Plan of Care and document tasks performed.
• Work with minimal supervision and be self-directed.
• Demonstrate dependability, tact, patience, and follow instructions.
• Communicate effectively with clients and team members.
• Experience in cleaning, laundry, medication reminders, escort services.

Responsibilities

• Observe client functioning and report changes to supervisor.
• Attend to client requests promptly and assist with activities of daily living.
• Provide companionship, socialization, and light housekeeping.
• Assist with medication reminders, provide prompts and container handling as needed.
• Plan meals, shop for food, and prepare meals.
• Maintain a clean, safe home environment; perform laundry, vacuuming, and dusting.
• Change bed linens and adhere to infection control policies.
• Document care and observations accurately for the home services coordinator.
• Communicate with supervisory staff and interdisciplinary team; protect confidentiality.
• Attend in-service trainings per agency policy.
