Job summary

All About You! Collaborative Health Care Services LLC. is seeking a Home Health Aide to provide compassionate care in the patient's home under the supervision of a Registered Nurse.

The position follows a fixed schedule: weekdays 3:00 PM–6:00 PM and weekends 8:00 AM–10:00 AM. The role includes assisting with personal care, mobility, routine exercises, and basic measurements like temperature, while documenting care and reporting any changes to the supervising RN.

Qualifications

• Certificate from state homemaker/home health aide training and competence evaluation program.
• Certificate from nurse's aide training program as defined by CT regulations.
• Ability to provide personal care under supervision with basic clinical tasks.

Responsibilities

• Assist the patient with personal care activities including bathing, oral hygiene, feeding and dressing.
• Assist with exercises, ambulation, transfer activities and medications that are ordinarily self-administered.
• Take and record temperature; observe and report patient status.
• Record care provided and communicate changes to the supervising RN.
• Perform activities under nursing supervision after training and observation.
