United Cerebral Palsy of Georgia, Inc. is seeking a part-time Host Home Case Manager in Atlanta, GA. The role oversees ISP implementation, documentation, and training for home providers to meet agency and DBHDD standards.

The ideal candidate will have at least two years of experience with intellectual and developmental disabilities, strong attention to detail, and comfort with travel across Georgia. Heavy documentation and compliance management are key parts of the job.

Qualifications

• Two years’ experience working with individuals with intellectual and developmental disabilities.
• Minimum two years’ experience working with individuals with intellectual and developmental disabilities.
• Comfortable with consistent travel throughout the state of Georgia.

Responsabilités

• Oversee ISP implementation and documentation for host home placements.
• Monitor goals, supervision, and person-centered outcomes for individuals.
• Review Therap documentation including shift notes and health tracking.
• Attend ISP, interagency, and DBHDD meetings as requested.
• Communicate quarterly with individuals’ families and natural supports.
• File and maintain Therap documents electronically and ensure accuracy.
• Coordinate visits, insurances, and medical records routing to nurses/coordinators.
• Ensure monthly paperwork is submitted by the 5th of each month.
• Provide training and guidance to home providers on documentation expectations.
• Conduct monthly field-day home visits to monitor service quality.

Formation

Bachelor’s degree in human services or related field
