InCommunity in Atlanta, GA seeks a Host Home Coordinator to manage and support Host Home arrangements for adults with disabilities. You will coordinate care, support Providers, monitor compliance, and facilitate communication with individuals, families, and internal teams.

This role emphasizes safety, person-centered planning, and regulatory adherence, with on-site travel across Metro Atlanta. A drive-ready candidate with a high school diploma and some related experience is preferred.

Qualifications

• High school diploma or equivalent preferred.
• At least six (6) months of experience in developmental disabilities, mental health, or aging services preferred.
• Personal or volunteer experience supporting individuals with disabilities may be considered.
• Valid driver’s license and meet company driving standards, if applicable.

Responsabilités

• Supervise and support Host Home Providers to ensure quality care.
• Recruit, interview, and onboard qualified Providers and Direct Care Staff.
• Coordinate training and development opportunities for Providers.
• Monitor service delivery and ensure ISP alignment and person-centered planning.
• Maintain regulatory compliance and confidential documentation.
• Provide weekly program updates to leadership.
• Collaborate with internal teams to improve processes.
