Legacy Hospice in Winchester, VA is seeking a dedicated Licensed Medical Social Worker (LMSW) to deliver compassionate social work services to hospice patients and families. The role focuses on counseling, emotional support, and care coordination in a patient-centered environment.

Schedule: Part Time - 24 Hours per Week, Coverage Area: Frederick, Clarke, Warren and Shenandoah. Requires MSW; LMSW preferred; 1 year hospice or healthcare experience; current driver's license.

Qualifications

• MSW from an accredited school of social work.
• Licensed Social Worker (preferred).
• Current driver's license and vehicle insurance.

Responsibilities

• Assess psychosocial status and coordinate with the RN Case Manager.
• Evaluate home situations, finances, and community resources.
• Develop individualized social work plans and collaborate with Bereavement Coordinator.
• Participate in interdisciplinary team meetings and support patients and families.
• Provide counseling with compassion to patients and families.
