Job summary

CorsoCare Hospice is seeking a dedicated Hospice Social Worker (LMSW) to deliver comprehensive social work services to patients and families. This role includes psychosocial, spiritual and bereavement assessments, counseling, outreach and referrals within the Oakland County area.

The position requires MSW from an accredited program, with preferred hospice experience. Responsibilities include developing care plans, crisis intervention, and collaboration with the IDG; travel between sites may be

Qualifications

• MSW degree from a CSWE-accredited program; one year of hospice care experience preferred.
• Ability to work independently and make accurate, quick judgments.
• Ability to respond appropriately to crises outside of hospital settings.
• Acceptance of and adaptability to different social, racial, cultural and religious modes.
• Minimum 2 years of social worker experience preferred.
• Active patient contact within past three years preferred.

Responsibilities

• Performs initial psychosocial, emotional, spiritual and bereavement assessments and assists in the development of IDG care plan.
• Conducts ongoing reassessments of patient/family needs and counseling as required.
• Provides short-term crisis intervention and individual or family counseling as indicated.
• Documents in the patient record per policy; maintains contact with community resources.
• Advocates for patient/family needs and facilitates referrals to services.
