Qualifications

• Associate Degree in Nursing and NM RN license are required.
• Minimum one year in direct patient care or home health care.
• Current NM driver's license and auto insurance. BLS required.

Responsibilities

• Direct patient care and case management for hospice patients.
• Coordinate care with a multidisciplinary team and physicians.
• Ensure timely assessments and documentation for regulatory compliance.
• Maintain plan of care and communicate changes to the team.
• Precepts and mentors when needed and manage schedule.
• Participate in QAPI and continuous improvement activities.
