Job summary

Interim HealthCare Inc. is seeking a Hospice and Palliative Care Nurse Practitioner to provide compassionate, patient-centered care in a full-time role.

You will perform bedside evaluations, assist with diagnosis and management of acute and chronic conditions, and collaborate with palliative care physicians to ensure high-quality care for our clients. The position requires an active Advanced Practice Nurse license and RN license in Indiana, plus a valid Controlled Substance license.

Qualifications

• Licensure: Current Advanced Practice Nurse and RN in the state of Indiana.
• Controlled Substance license required.
• Hospice or Palliative care experience preferred.

Responsibilities

• Perform bedside palliative care evaluation and assist with diagnosis, treatment and management of acute and chronic conditions.
• Consult with palliative care physicians and inform primary physician of services provided; collaborate as needed.
• Prescribe medications, including controlled substances within delegated authority.
• Prepare and maintain accurate patient records and documentation for sound medical practice and reimbursement.
