Job summary

VNA & Hospice of the Southwest Region in Vermont seeks a contracted Hospice Associate Medical Director for Bennington County, offering 5-10 hours per week. You will provide medical direction to the hospice program and ensure continuity of care within an interdisciplinary team.

Responsibilities include overseeing physician services, coordinating with attending physicians, and supporting quality improvement efforts while acting as a community liaison to promote hospice and palliative care.

Qualifications

• Licensed MD or DO in Vermont without restrictions.
• Maintain controlled substances registration.
• Experience in hospice or palliative care.
• Active participation in medical education related to hospice care.
• Valid Vermont nursing or therapy license and driver's license; background checks required.

Responsibilities

• Provide overall medical direction to the hospice program and staff.
• Review patient eligibility and collaborate on the plan of care with the team.
• Ensure compliance with Medicare Conditions of Participation and state/federal laws.
• Liaise with community providers and discharge planners for admissions.
• Lead QAPI initiatives and participate in education and quality improvement.
• Serve as hospice champion in the community.
