Job summary

Liberty Cares in Blowing Rock, NC is seeking a part-time Hospice Licensed Practical Nurse (LPN) to provide compassionate end-of-life care. You will work with the Hospice team to deliver direct patient care, document per policy, and follow the established plan of care under the supervision of the RN.

Responsibilities include coordinating services with the RN and care team, accepting physician orders, and contributing to IDT meetings.

Qualifications

• Graduation from an approved school of nursing.
• Current LPN licensure in the state of practice.
• One year of clinical nursing experience preferred.
• Strong communication and documentation skills.

Responsibilities

• Provides direct patient care, including coordination of services.
• Completes documentation per policy and regulation.
• Follows care plans as directed by the RN.
• Coordinates Hospice services with the RN and care team.
• Accepts and transcribes physician orders.
• Attends and participates in IDT meetings.
