Job summary

Choice Health at Home is seeking a licensed LMSW to join our hospice team in Athens, TX. The Social Worker provides emotional, social, and practical support to patients and families, collaborating with an interdisciplinary team to coordinate home-based care.

Responsibilities include completing comprehensive psychosocial assessments within 5 days of election of hospice, assisting with advance directives and discharge planning, and offering end-of-life counseling and resources.

Qualifications

• Bachelor’s degree in Social Work from an accredited school.
• One or more years of Hospice experience required.

Responsibilities

• Assesses and completes the Comprehensive Assessment within 5 days of the patient’s election of hospice care to identify psycho-social, financial, environmental, and community needs.
• Assists with advance directives, funeral planning, discharge planning, and family counseling as indicated by patient/family needs.
• Provides patient and family/caregiver counseling around end-of-life, death, and grief.
• Attends interdisciplinary team meetings and coordinates with the IDG to ensure coordinated home-based care.
