Job summary

Optum is seeking a part-time Home Infusion Nurse to deliver clinical care and education to home-based patients with infusion therapies. You will perform nursing assessments, manage IV access, administer medications, and document care as per orders in AIS or home settings.

You must hold an active CA RN license, have 1+ year in home infusion, and be comfortable with flexible 20-hour weeks, travel, and collaborative practice with a multi-disciplinary team.

Qualifications

Active and unrestricted RN license in California.
1+ year RN experience in home health or infusion settings.
Proficient with electronic medical record documentation.
Able to work part-time 20 hours per week and travel as needed.

Responsibilities

Provide infusion therapy and patient education in home or AIS settings.
Complete clinical documentation including POC/POT, notes and summaries.
Monitor patient response and coordinate with the care team.
Establish vascular access and administer prescribed medications as ordered.
Collaborate with HHAs, pharmacists, physicians and other staff.
