Job summary

UnitedHealth Group is seeking a Home Infusion Nurse to provide one-on-one, skilled clinical care at patients' homes or ambulatory infusion sites. You will work with a multidisciplinary team, receive comprehensive training, and deliver safe, compassionate infusion care with autonomy and a flexible schedule.

Role includes travel within Colgate and Brookfield areas, 40 hours per week (drive time included), and a 1:1 patient connection to see the impact of your nursing care firsthand.

Qualifications

• Active unrestricted RN license in the state of MN.
• Current BLS/CPR certification or willingness to obtain prior to start.
• Comfortable performing IV starts in diverse patient populations.
• Ability to work independently with solid clinical judgment.

Responsibilities

• Travel to patient homes or sites using a personal or rental vehicle; follow travel policies.
• Provide safe, compliant infusion services per physician orders and policies.
• Educate patients and caregivers on self-care and self-administration.
• Assess home suitability, caregiver support, and patient independence for home infusion.
• Maintain accurate patient histories, assessments, plans of care, visit notes, and documentation.
• Deliver skilled nursing care across patient ages per treatment plans.
• Manage vascular access devices (IVs, PICC, ports).
• Administer medications; monitor responses and manage reactions and labs.
• Communicate patient status with physicians, pharmacists, and care team.
• Assist in staff orientation and clinical training.

As a Home Infusion Nurse, you'll provide one on one, skilled clinical care to patients where they feel most comfortable-at home or in an ambulatory infusion suite. Primary Responsibilities

• Travel to patient homes and related sites using a personal or rental vehicle in accordance with company travel policies
• Provide safe and compliant infusion services in accordance with Optum Infusion Pharmacy policies, protocols, and physician order
• Establish and maintain positive therapeutic relationships with patients and caregivers, including education on self-care and self-administration
• Assess patient suitability for home infusion therapy, including home safety, caregiver support, and patient independence
• Obtain and maintain accurate patient medical histories, clinical assessments, and all required documentation, including plans of care, visit notes, medication profiles, and discharge summaries
• Deliver skilled nursing care across a range of patient ages and conditions based on physician orders and treatment plans
• Establish, access, and maintain vascular devices, including peripheral IVs, PICC lines, tunneled catheters, and implanted ports
• Administer medications and monitor patient responses, including managing infusion reactions, vital signs, side effects, and lab collection
• Communicate patient status and therapy response promptly with physicians, pharmacists, supervisors, and other care team members
• Assist in staff orientation and training, serving as a clinical educator and resource as requested

Required Qualifications

• Active and unrestricted Registered Nurse (RN) license in the state of MN
• Current BLS/CPR certification or willingness to obtain prior to start
• Comfortable and confident performing IV starts in a variety of patient populations
• Demonstrated ability to work independently with solid clinical judgment and effective interdisciplinary collaboration skills
• Valid driver's license and access to reliable transportation

Preferred Qualifications

• Home infusion experience
• 1+ years of RN experience in a clinical setting supporting infusion or IV therapy (e.g., ICU, Med Surg, ED, PACU)
• Familiarity with Microsoft Office and EMR platforms
• Knowledge of Infusion Nursing Society (INS) standards
• Understanding of HIPAA, OSHA, and home infusion accreditation guidelines
• Proven solid communication, organization, critical thinking, and teaching skills

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $29.00 to $52.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
