Job summary

Optum, part of UnitedHealth Group, is seeking a Home Infusion Nurse to provide one-on-one nursing care in patients' homes or ambulatory infusion sites. The role offers autonomy, a flexible schedule, and the chance to build meaningful connections while delivering life-changing therapies.

You’ll travel within a 2-hour radius of Salem, OR, work about 20 hours per week, and join a multidisciplinary team with comprehensive training.

Qualifications

• Active and unrestricted RN license in OR.
• Current BLS/CPR certification or willingness to obtain prior to start.
• 1+ years of RN experience in a clinical setting supporting infusion or IV therapy (e.g., ICU, Med Surg, ED, PACU)

Responsibilities

• Travel to patient homes and sites using a personal or rental vehicle as per policies.
• Provide safe and compliant infusion services per orders and policies.
• Establish and maintain positive therapeutic relationships with patients and caregivers; educate on self-care.
• Assess patient suitability for home infusion therapy, including safety and caregiver support.
• Document medical histories, assessments, plans of care, visit notes, and discharge summaries.
• Deliver skilled nursing care per physician orders and treatment plans.
• Establish and maintain vascular access devices (IVs, PICC lines, ports).
• Administer medications and monitor responses, including infusion reactions and vital signs.
• Communicate patient status and therapy response with physicians and the care team.
• Assist in staff orientation and provide clinical education as requested.
