Qualifications

• CCC-SLP or in process of accumulating supervised experience.

Responsibilities

• Assess, plan, and deliver speech-language pathology services in home health.
• Develop and revise care plans with physicians and clients.
• Educate clients and caregivers on treatment and recovery techniques.
• Document clinical findings, progress and updates per policy.
• Participate in quality improvement and interdisciplinary coordination.
• Supervise HHAs and mentor new staff as assigned.
