Job summary

IHA Monticello-Above and Beyond seeks a part-time Speech-Language Pathologist to provide diagnostic and therapeutic services in home health, in accordance with state licensure and physician prescriptions.

You will develop individualized treatment plans, educate clients and families, document findings, and collaborate with physicians and the care team to promote communication and independence. Travel between sites may be required.

Qualifications

• CCC or in process for CCC with supervised experience.
• Active state licensure to practice speech-language pathology.
• Ability to work with minimal supervision and adhere to policies.
• Reliable transportation and valid driver's license.

Responsibilities

• Assess, plan, and deliver speech-language pathology services in home health settings
• Develop and revise care plans in collaboration with physicians and clients
• Educate clients and caregivers on treatment and recovery techniques
• Document clinical findings, progress, and updates in accordance with agency policy
• Participate in quality improvement and interdisciplinary coordination
• Supervise HHAs and mentor new staff as assigned
• Perform assessments, visit reports, and coordinate data for client records
