Qualifications

• Must hold a current Massachusetts Physical Therapist license.
• Prior home health care experience is not required.
• Part-Time schedule with flexible work-life balance.
• Work with adult and geriatric patients in their homes in Somerset and Fall River.

Responsibilities

• Making visits to clients in their homes.
• Perform diagnostic tests and measurements, such as mobility, transfer status, balance and safety assessments.
• Develop and implement individualized care plans, including manual therapy exercises, gait training and balance interventions.
• Continually assess and revise the Physical Therapy care plan and coordinate with nursing on the multidisciplinary plan.
• Educate clients and family members in rehabilitative care to promote health, safety and independence.
