Job summary

Elara Caring in Fort Worth, TX is seeking a Part-time Physical Therapist to join our home health team. You will evaluate patients, develop individualized treatment plans, and deliver therapy in patients' homes.

The role requires graduation from an APTA-approved program, an active PT license, at least one year in a healthcare setting, and reliable transportation. You will document care, collaborate with physicians, and supervise therapy assistants as appropriate.

Qualifications

• Graduate of an APTA approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one (1) year of experience in a healthcare facility.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services in the home setting.
• Monitor patient progress and adjust care plans as needed.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.
• Complete timely and accurate clinical documentation.
• Supervise therapy assistants and support staff, as appropriate.
