Job summary

Elara Caring is seeking a Physical Therapist to deliver in-home care in the Fort Worth, TX area. You will evaluate patients, develop individualized treatment plans, and provide therapy services in patients' homes, collaborating with a multidisciplinary team.

Requirements include an APTA-approved PT program graduate, current Texas PT license, and at least 1 year of healthcare facility experience. A valid driver's license or reliable transportation is needed to travel between patient homes.

Qualifications

• Graduate of an APTA approved physical therapy program.
• Current, unrestricted PT license valid for the state of work (Texas).
• Minimum one year of experience in a healthcare facility.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services in the home setting.
• Monitor patient progress and adjust care plans as needed.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.
• Complete timely and accurate clinical documentation.
• Supervise therapy assistants and support staff, as appropriate.
