Job summary

Elara Caring is seeking a Physical Therapist to deliver high-quality, in-home care across Texas. You will assess patients, design and implement plans of care, and collaborate with clinicians to advance our home-based care model.

Requirements include an APTA-approved PT program, current PT license, and at least 1 year in a healthcare facility. Willingness to travel up to 50% and a dependable vehicle are required.

Qualifications

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Able and willing to travel within branch/office coverage area.
• Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
• Must be able and willing to travel 50%.

Responsibilities

• Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists.
• Designs and implements a plan of care for patients based on a thorough assessment.
• Directs the assistance of support staff to achieve patients’ functional goals.
• Documents evaluations, treatment goals, and plans; maintains prescriptions and signatures.
• Provides leadership to staff and promotes physical therapy in the community.
