Job summary

Elara Caring in Fort Worth, TX is seeking a Physical Therapist to deliver high-quality home health services. You will evaluate patients and develop individualized treatment plans to improve function and independence in the home setting.

Responsibilities include documenting patient progress, collaborating with physicians and caregivers, and supervising therapy assistants as appropriate. APTA-approved program, active PT license, and valid driver's license are required.

Qualifications

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one (1) year of experience in a healthcare facility.
• Must possess a valid driver's license and reliable transportation to travel between patient homes.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services in the home setting.
• Monitor patient progress and adjust care plans as needed.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.
• Complete timely and accurate clinical documentation.
• Supervise therapy assistants and support staff, as appropriate.
