Job summary

Alternate Solutions Health Network is seeking a part-time Occupational Therapist (OT) to join our home health team serving Brown, Clermont, Clinton, Hamilton, Highland, and Warren Counties.

You'll provide comprehensive skilled OT services as ordered by the attending physician, educate patients and families, document progress, and collaborate with a multidisciplinary team to support safe, effective recovery in patients’ homes. A valid Ohio OT license and driver’s license are required.

Qualifications

• Current license as Occupational Therapist in the state of employment.
• Minimum one year experience as OT in an acute care setting.
• Home care experience preferred.
• Valid driver’s license and auto insurance in your name as a driver.
• Capable of all physical demands.

Responsibilities

• Plan of Care: provide skilled OT as ordered by attending physician; observe, record, and report patient progress.
• Evaluation: assess patient functioning and prognosis.
• Patient Outcomes: use home exercise programs to improve ROM, strength, and coordination.
• Minimize Patient Risk: reduce re-hospitalizations by teaching preventive measures.
• Family Educator/Advocate: educate patient and family on disease/injury management and equipment use.
• Supervision: supervise COTA during total care.
• Communication: coordinate with COTA on supervisory visit results.
• Documentation: complete clinical documentation per agency and Medicare guidelines.
• Regulatory Compliance: follow agency policies and notify schedule changes.
• Professional Development: attend required in-service trainings.
