Job summary

Graham Healthcare Group is seeking a Home Health Occupational Therapist in Skokie, Illinois to provide patient-centered occupational therapy services in a home-health setting. The role is 32 hours per week with a staff employment arrangement and a competitive pay rate.

You will assess and treat patients, develop plans of care, document progress in EMR, and coordinate with caregivers and other clinicians. A BS or MS in Occupational Therapy plus state licensure and at least one year of healthcare

Qualifications

• Must hold a Bachelor of Science or Master’s degree in Occupational Therapy (OT).
• Licensed Occupational Therapist in the state where you will work.
• Minimum of one year healthcare experience.
• Valid driver’s license and ability to travel ~20% of time.

Responsibilities

• Provide home health OT services to patients in assigned areas.
• Assess, plan and implement individualized OT for daily living and functional goals.
• Document patient progress in EMR and coordinate with care team.
