Job summary

Graham Healthcare Group is seeking a Home Health Occupational Therapist to join our Residential Home Health and Hospice team in Medinah, Illinois. This full-time, staff position offers ambitious pay and a comprehensive benefits package.

The OT will collaborate with a care team to develop and implement patient care plans, provide therapy services in home settings, and travel to patients' locations as needed. A valid OT license and driving flexibility are required.

Qualifications

• Bachelor’s or Master’s degree in Occupational Therapy from an accredited program.
• Licensed Occupational Therapist in the state of Illinois.
• Minimum one year healthcare experience.
• Valid driver’s license with ability to travel to patient locations (~20%).

Responsibilities

• Develop and implement patient care plans in home settings.
• Collaborate with a care team to manage patient therapy.
• Provide OT services and document progress in EMR.
• Travel to patient locations as required.
