Job summary

Canyon Home Care & Hospice LLC in Gilbert, AZ is seeking a Part Time Occupational Therapist to provide intermittent home-based OT under physician orders and supervision. You will evaluate patients, develop and revise plans of care, and coordinate with physicians and families.

Experience and licensure requirements include state OT licensure, ACOTE-accredited education, and NBCOT eligibility, with two years of experience preferred. CPR certification is required.

Qualifications

• Licensed or eligible as an OT in the practicing state.
• Graduate of ACOTE-accredited OT education program.
• Eligible to take NBCOT certification exam.
• Two years of OT experience preferred.

Responsibilities

• Assists physician in evaluating function and plan of care.
• Prepares clinical and progress notes.
• Advises family and agency personnel.
• Participates in in-service programs and training.
• Completes OASIS assessments and related documents.
• Performs all OT procedures as ordered by physician.
• Maintains confidentiality and timely documentation.
• Ensures adherence to agency policies and teamwork.
