Job summary

Canyon Home Care and Hospice seeks an Occupational Therapist to provide in-home therapy under physician orders and with supervision from the Clinical Manager/Therapy Supervisor. You will perform skilled OT procedures, complete evaluations, and coordinate with family and care team in patients’ homes.

Candidates should hold appropriate licensure and meet ACOTE/NBCOT prerequisites; prior home health experience is a plus.

Qualifications

• Licensed or regulated as an occupational therapy assistant in the practicing state.
• ACOTE-accredited OT assistant education program completion.
• NBCOT entry-level certification eligibility.

Responsibilities

• Assist physician in evaluating function and developing the plan of care.
• Prepare clinical and progress notes.
• Provide home-based occupational therapy per physician orders.
• Complete OASIS assessments and transfers when trained.
• Document and report patient progress to physicians.
• Maintain confidentiality and comply with agency policies.
