Job summary

Assisted Home Health & Hospice seeks an Occupational Therapist (OT) for Home Health in the East Valley, serving Phoenix, AZ. Both full-time and part-time opportunities are available. Join a team dedicated to improving function, reducing pain, and promoting independence in daily living activities.

You will perform initial and ongoing assessments, design treatment plans, and collaborate with physicians and care partners to support patient goals and safety at home.

Qualifications

• Graduate of an OT curriculum and licensed to practice.
• At least one year of experience in occupational therapy.
• Current CPR, TB and health clearance.

Responsibilities

• Complete initial and ongoing interdisciplinary assessments with emphasis on level of function and response to therapy.
• Evaluate function and initiate a plan of treatment to relieve pain, develop skills, restore function and maintain maximum capabilities.
• Initiate self-care modifications and teach patients and families to use daily living aids.
• Develop and review the plan of care with physicians and the care team, in partnership with patients and caregivers.
• Provide therapeutic treatments and evaluate equipment needs as ordered by the physician.
• Communicate with all physicians involved in the plan of care and other practitioners as appropriate.
• Perform other duties as per job descriptions.
