Job summary

Angels Care Home Health is seeking a qualified Occupational Therapist to join our dedicated home health team in Arizona. You will conduct clinical assessments, develop and revise the Plan of Care, and document progress notes to ensure high-quality patient outcomes.

The role requires an active OT license, at least two years of experience, and current CPR/Hepatitis status. You will work with multidisciplinary staff and May coordinate with durable medical equipment needs in patient care.

Qualifications

• Must hold current state OT license.
• At least 2 years of experience as an Occupational Therapist.
• CPR certification and Hepatitis profile current.
• Knowledge of durable medical equipment (DME).

Responsibilities

• Perform clinical assessment and identify patient needs and appropriateness of care.
• Document, prepare, and complete a Plan of Care, progress notes, and other clinical records.
• Revise the Plan of Care based on ongoing evaluation.
• Apply infection control and universal precautions in patient care.
• Assess physical, social, and cultural components of the patient’s environment.
